In patients with inflammatory bowel disease presenting with acute abdominal pain, local peritoneal signs and mild neutrophilia, how does one distinguish exacerbation of IBD, acute enterocolitis and surgical etiologies? JEAN-PIERRE LEUNG, MD, Calgary, AB
Local tenderness and neutrophilia may just be a flare-up of Crohn’s disease. But in severe ulcerative colitis, local tenderness should prompt a high index of suspicion for a perforation. Other possibilities include unrelated conditions such as acute diverticulitis.
