Does lack of hep B antibodies mean lack of immunity?

If someone had hep B vaccination in school and 5 years later shows no antibodies, do they need reimmunization? S. REMTULLA, MD, Mississauga, ON

A three-dose course of hepatitis B vaccine will result in protection from infection in ≥ 95% of healthy individuals. Any post-vaccination anti-HBs titre ≥ 10 mIU/ml (even if it later falls below that level) is generally accepted to represent long-term protection from disease. Given the high rate of response to hepatitis B immunization, routine testing for serologic response isn’t usually performed in individuals who aren’t high-risk. If your patient had a previously documented post-vaccination anti-HBs titre ≥ 10 mIU/ml, then no further routine immunization is required in the absence of a high-risk exposure. If this is unknown, then they are potentially at risk and a repeated three-dose series is reasonable.
