Painful prescribing dilemma
June 2009
What’s the recommended response to a brief urgent care visit from a patient with chronic pain (Workplace Safety and Insurance Board file brought with him) on oxycodone 80 mg 3 tabs po tid, requesting a medication repeat after his family doctor lost narcotic prescribing privileges? A. SORENSON, MD, Oshawa, ON
I assume the problem is that this patient is only coming to you to get a prescription, and not to become your patient or to obtain a second opinion. Your Code of Ethics instructs you to provide appropriate assistance to a person with an urgent need. I believe the key word here is “appropriate.” If, after reading the patient’s file and examining him, you feel it’s appropriate to repeat the medication, I see no reason not to do so. It would be irresponsible and unethical, however, to simply rubber-stamp the treatment given by a colleague.
